Immunizations and Health History

New York State law and/or New York University policy requires that all incoming students complete several health related requirements that include a health history, proof of immunization to measles, mumps and rubella, and an acknowledgement that you have received information regarding meningitis. Enrollment will not be finalized for students who have not complied with the immunity law and policy.

All students are required to complete Student Health History and Immunization forms. These forms are provided in your admissions pack. You are required to complete these forms no later than August 15, 2011 (August 3 for Summer Start) to enroll at NYU Stern. If you do not complete these forms by August 15 (or August 3 for Summer Start), your registration will be canceled and you will not be permitted to attend classes.

Learn more about the health history and immunization requirements for new graduate students on the NYU Health Center website.
