New York State Immunization Requirement
New York State Public Health Law 2165
Please review this important immunization information carefully.
This law requires that all students attending colleges and universities, living on and off campus or studying abroad, who are registered for 6 or more credits, and who were born on or after January 1, 1957, must prove that immunization requirements for MMR (Measles, Mumps and Rubella) have been met.
New York State Public Health Law 2167
This law requires institutions, including colleges and universities, to distribute information about Meningococcal disease and vaccination availability to all enrolled students, living on or off campus or studying abroad, who are registered for 6 or more credits. A response to receipt of Meningococcal disease and vaccine information, signed by the student or a student's parent or guardian (if student is under the age of 18), is required.
All students are required to complete the Health Records forms before Langone orientation (the Health Records forms will be included in your admissions pack). You must return this form directly to the NYU Health Center.*

Students who have not completed the entire form will not be permitted to attend classes.

NOTE: If you have already completed the MMR and Meningitis Response PDF forms previously posted on the Langone Part-time Admitted Students website and sent them in, complete the Health History portion when you receive it and submit the entire packet to Health Services. Do not detach the MMR or Meningitis Response form from the Health History.
