Personal Information

Please note the following when completing the Personal Information Parts I and II of the application.

Social Security Number

Social Security Numbers should be submitted by U.S. citizens and U.S. Permanent Residents. If you are an international student who has been assigned a Social Security Number while studying or working in the U.S., you should also provide us with this information. Otherwise, you should leave this blank.

Telephone/Mobile/Cell Numbers

We may need to contact you via phone. Please provide these numbers only if we may use them to contact you. If your contact information changes at any time, you are responsible for providing the new information to MBA Admissions immediately via the Application Status Check Website.

Citizenship

Please indicate your citizenship status: U.S. citizen, U.S. Permanent Resident, citizen of another country or dual citizen. If you are a U.S. Permanent Resident, please indicate your country of citizenship.

College Code

College Code information is provided in the GMAT or GRE bulletin, which is sent to you upon requesting a test registration form via www.MBA.com or www.taketheGRE.com.

Company Sponsorship

Langone Part-time MBA students who are working full-time may have their employer sponsor part or all of their tuition and fees. Sponsorship does not affect your chances of admission. Company sponsorship is considered financial aid when the company sends the funds to NYU Stern to be directly applied to your tuition bill; it will be counted into your Cost of Attendance and may affect your financial need. You should inquire with your employer as to whether sponsorship is available for you. You are asked to enter the maximum amount per year that your employer will sponsor.

Intended Academic Specialization(s)

Your selection of intended academic specialization(s) is not binding. Indication of an academic specialization is encouraged, but not required.

Armed Forces Information

Please indicate if you are on active duty, a reserve member or veteran of the armed forces.

Racial/Ethnic Background
